The Sangamon County Department of Public Health is seeking applicants to perform duties primarily as an Early Intervention Service Coordinator.
Responsibilities* Coordinate services and support efforts in a timely manner to families who are within the early intervention process.
* Develop and aid family with individualized family service plans (IFSP) that meet family's priorities and strengths.
* Develop and maintain strong relationship with families within the program offering transition options and assisting in the development of a transition plan.
* Maintain extensive documentation and ensure timely submission of required paperwork.
* Provide service coordination ensuring all qualified children under age 3 receive ongoing early intervention service delivery at the Child and Family Connections site for Region 18.
* Schedule and maintain intakes, evaluations and assessments with eligible families.
* Attend seminars, workshops, and educational trainings related to early intervention for professional growth and grant requirements.
* A valid Class D Illinois Driver's License
* Applicants must be a resident of Sangamon County or be willing to move into the County within 6 months of employment
* Ability to use personal automobile with proof of current insurance
* Bachelor's degree in human services, behaviorial science, social science or a health related field with experience in a community based social services program or a current State of Illinois registered nursing license.
* Ability to collaborate and maintain relationships with relevant stakeholders
* Attention to detail
* Proficient writing and reading abilities
* Strong organizational skills
Work hours are typically Monday-Friday 8a-430p. Salary: $51,213.40/yr-$64,016.75; 2 weeks paid vacation, 3 paid personal days, 12 paid sick leave days and 13 paid holidays per year. Individual and Family health, dental and life insurance options, retirement and mileage reimbursement.
